Health Services & Medical Forms: Deadline May 15 (1st Summer Session); June 12 (2nd Summer Session)

Please go to the Student Health Services site for updated information on medical forms, immunizations that you may need to do prior to the start of your summer courses. This site also contains the hours and services that may be available for you during the summer.

Health Insurance Cards: Deadline April 18

ONLY Modular Students, including International & Canadian

  • Please review the Student Health Insurance Card information in the Health Services site for important information relating the health insurance.
  • Send a copy of your health insurance card to All modular students who are registered for classes must send a copy of their insurance card to Student Health Services. This proof must be a copy of the current health insurance card and can be in the form of a fax, email, or traditional mail. If a student does not have health insurance or fails to comply with this requirement, a fee anywhere between $150-$200 will be charged to their student account. This fee will cover the cost of health insurance for them while on campus. Temporary insurance is accepted but a copy of the insurance card is still required. The proof of insurance must be sent to Student Health Services each time you come to campus. For more information contact
  • International (including those from Canada) modular students must purchase insurance through Student Health Services each time you come to campus. The insurance cost is billed at a daily rate for a minimum of ten days. For more information, email
