Student Health Insurance Claims
If you are on the Student Health Insurance plan, you must submit a claim form to the insurance company with any new illness or injury. You may print a claim form out and mail it in for any medical services you have completed. For more information regarding claims, please go to www.Klais.com >>

Athletic Sports Insurance
If you are a varsity student-athlete injured during the official season of play, you must fill out the appropriate claim form. Please contact the Athletic Training Office at 630-752-5738 for further assistance. The claim form and letter are listed below for your convenience.
